MetroPlusHealth Managed Long-Term Care is a health care plan especially designed for people 21 years or older, who live in the Bronx, Brooklyn, Queens, Manhattan, and now Staten Island who need long-term care services, and have Medicaid. MetroPlusHealth Managed Long-Term Care offers the assistance you need to live safely at home.

You Are Eligible if You Are…
21 years old or older
Eligible for Medicaid
Living in the Bronx, Brooklyn, Manhattan, Staten Island or Queens
In need of long-term care or nursing home-level care
Able to remain in your home without jeopardizing your health or safety
In need of long-term care services for at least four months from the time of enrollment

Your Care Plan:
When you join MetroPlusHealth Managed Long-Term Care, you will be assigned a Care Manager. Your Care Manager will work with you, your family, your primary care physician (PCP) or others that you choose to work on your behalf. Your Care Manager will develop an individual care plan for you.
Your Care Manager will develop a care plan that is specific to meeting your individual medical, physical, emotional, and social needs, based on the initial comprehensive assessment. Your written care plan covers the services you need, how often you need them and how long you will need them for. Your care plan is reviewed at least every six months, but can be reviewed more often if your condition and needs change. You will receive a copy of your care plan.

- Durable Medical Equipment, including Medical and Surgical Supplies, Prosthetics, Orthotics, and Orthopedic Footwear
- Eye exam and eyeglasses
- Foot Care
- Hearing Exams and Hearing Aides (including hearing aide batteries)
- Home Care including nursing, home health aides, rehabilitation therapies, and medical social work
- Home Delivered or Congregate Meals
- Home Safety Modification/Improvement
- Hospice
- Long-Term Placement at a Veterans’ Home
- Nursing
- Nursing Home Care (certain Medicaid restrictions apply)
- Nutritional Counseling and Nutritional Supplements
- Personal Care
- Personal Emergency Response System (PERS)
- Private Duty Nursing
- Rehabilitation Therapies (Physical, Occupational, and Speech Therapy) provided in Setting other than Home
- Respiratory Therapy
- Respiratory Therapy and Oxygen
- Scheduled Transportation to Health related Appointments
- Social Adult Day Care
- Social and Environmental Support
- Telehealth

When you get emergency care or are treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from balance billing. In these cases, you shouldn’t be charged more than your plan’s copayments, coinsurance and/or deductible. Learn more here.

Manage Your Health Online With MyChart. Members who receive care at NYC Health + Hospital facilities now have access to MyChart. MyChart is a free secure portal you can use to manage your health information.

GREAT NEWS: STARTING JANUARY 1, 2023, METROPLUSHEALTH MEMBERS WILL RECEIVE THEIR DENTAL BENEFITS FROM DENTAQUEST.
Recently, we sent you an announcement along with your new ID Card about our new partnership with DentaQuest, a dental vendor. Starting January 1, 2023, you can begin to use your new ID Card for all your health care needs, including dental services with DentaQuest. Remember to bring your new ID card when you visit your dentist.
