MetroPlusHealth Managed Long-Term Care is a health care plan especially designed for people 21 years or older, who live in the Bronx, Brooklyn, Queens, Manhattan, and now Staten Island who need long-term care services, and have Medicaid. MetroPlusHealth Managed Long-Term Care offers the assistance you need to live safely at home.

You Are Eligible if You Are…

  • 21 years old or older

  • Eligible for Medicaid

  • Living in the Bronx, Brooklyn, Manhattan, Staten Island or Queens

  • In need of long-term care or nursing home-level care

  • Able to remain in your home without jeopardizing your health or safety

  • In need of long-term care services for at least four months from the time of enrollment

Your Care Plan:

When you join MetroPlusHealth Managed Long-Term Care, you will be assigned a Care Manager. Your Care Manager will work with you, your family, your primary care physician (PCP) or others that you choose to work on your behalf. Your Care Manager will develop an individual care plan for you.

Your Care Manager will develop a care plan that is specific to meeting your individual medical, physical, emotional, and social needs, based on the initial comprehensive assessment. Your written care plan covers the services you need, how often you need them and how long you will need them for. Your care plan is reviewed at least every six months, but can be reviewed more often if your condition and needs change. You will receive a copy of your care plan.

  • Home Care including nursing, home health aides, rehabilitation therapies, and medical social work

  • Long-Term Placement at a Veterans’ Home
  • Nursing
  • Nursing Home Care (certain Medicaid restrictions apply)

  • Personal Emergency Response System (PERS)
  • Private Duty Nursing
  • Rehabilitation Therapies (Physical, Occupational, and Speech Therapy) provided in Setting other than Home

MetroPlusHealth Virtual Visit is the fast, easy way for our members to see a provider for common physical, mental, emotional, and behavioral health issues that are not emergencies. MetroPlusHealth Virtual Visit is available anytime, from anywhere in New York State, 24/7 – and in your language.
