Please note: Our network is now open to Radiology Facilities and Non-Emergent Transportation.

Please note: The network is closed to Podiatry, Physical Therapy, Radiology, Home Infusion, Optometry, Laboratory, Urgent Care, Licensed Home Care Agency/Personal Care Services,  Certified Home Care Agency, Consumer Directed Personal Assistance Program (CDPAS) (in these areas: Bronx, Brooklyn, Queens, and Manhattan).  Consequently, we are not accepting applications for these specialties.  The network is open to Optometry, Podiatry, Physical Therapy, Transportation and Urgent Care in Staten Island.
